Hypnotic drug risks of mortality, infection, depression, and cancer

This is a review of hypnotic drug risks and benefits, reassessing and a significantly-increased adjusted risk of cancer mortality for zolpidem.

Of studies analyzing follow-ups of 8 years or less, 22 of 26 studies reported a significant association, but of studies with longer follow-ups, only 10 of 14 studies observed significant mortality risks. Mixing weakens the risk-ratio contrasts observed. In long follow-ups, one is also studying the selected survivors of the more marked short-term risks that have been recently described 14, 15. This may suggest that during long prospective follow-ups, many patients initially taking hypnotics will discontinue hypnotic usage, whereas many controls not using hypnotics at prospective baseline may have begun using hypnotics during a long follow-up, so that the longer the follow-up, the more mixing of hypnotic-consuming and control groups becomes likely.

The behavioral profile of zolpidem is consistent with the suggestion that this compound may have selective sedative action produced by activity at a sub-type of benzodiazepine receptor. The imidazopyridine zolpidem has previously been shown to displace benzodiazepines from their receptors, with a preferential activity at BZ 1 sites, and to exert hypnotic activity in man. These differences appeared to be qualitative rather than quantitative with the stimulus properties of zolpidem being related to the drug's sedative action. In conditions where tolerance developed rapidly to the depressant activity of benzodiazepines little tolerance was seen with zolpidem. However, unlike benzodiazepines, zolpidem exerts sedative actions at relatively low doses. In drug discrimination experiments differences between the internal stimuli produced by chlordiazepoxide and zolpidem were identified. Zolpidem's pharmacological profile includes anticonvulsant, antipunishment and behavioral depressant actions.
