Cognitive behavioral therapy alone was superior to sertraline in both adults and children; however, the best results were achieved using a combination of these treatments. Sertraline may be useful for the treatment of OCD co-morbid with Tourette syndrome.

As with most other SSRIs its toxicity in overdose is considered relatively low. Acute overdosage is often manifested by emesis, lethargy, ataxia, tachycardia and seizures. Plasma, serum or blood concentrations of sertraline and norsertraline, its major active metabolite, may be measured to confirm a diagnosis of poisoning in hospitalized patients or to aid in the medicolegal investigation of fatalities.

Its chief active metabolite is norsertraline ( N - desmethylsertraline ) which is significantly less biologically active than its parent compound.

Despite being a more potent dopamine reuptake inhibitor than bupropion, it is still a much more potent inhibitor of serotonin reuptake — it is about 60 times more potent at inhibiting serotonin reuptake than it is at inhibiting dopamine reuptake.

In 281 pediatric patients treated with ZOLOFT in placebo-controlled studies, the overall profile of adverse reactions was generally similar to that seen in adult studies. Adverse reactions that do not appear in Table 3 (most common adverse reactions in adults) yet were reported in at least 2% of pediatric patients and at a rate of at least twice the placebo rate include fever, hyperkinesia, urinary incontinence, aggression, epistaxis, purpura, arthralgia, decreased weight, muscle twitching, and anxiety.

False-positive test results may be expected for several days following discontinuation of ZOLOFT.
