When a drug is being used to induce sleep or treat a sleep disorder, the facility should attempt periodic tapering of the medication or provide documentation of medical necessity in accordance with OBRA guidelines. In addition, the Beers expert panel recommends avoiding zaleplon in elderly patients with a history of falls or fractures, unless safer alternatives are not available, since zaleplon can produce ataxia, impaired psychomotor function, syncope, and additional falls; if zaleplon must be used, consider reducing use of other CNS-active medications that increase the risk of falls and fractures and implement other strategies to reduce fall risk.

Doses above 20 mg have not been adequay evaluated and are not recommended. The dose of Sonata should be individualized. The recommended dose of Sonata for most nonelderly adults is 10 mg. For certain low weight individuals, 5 mg may be a sufficient dose. Although the risk of certain adverse events associated with the use of Sonata appears to be dose dependent, the 20 mg dose has been shown to be adequay tolerated and may be considered for the occasional patient who does not benefit from a trial of a lower dose.
